Forever Home ™
Our Forever Home™
program provides frail, elderly persons with the necessary assistance in
order for them to remain living at home and to prevent nursing home
placement.
This program began in 2004 and currently serves over 350 clients. It is
designed for seniors age 65+ who need assistance with at least three of
their Activities of Daily Living (ADL), or who have been diagnosed with
Alzheimer's disease or other dementia-related illnesses.
Services include: home delivered meals, homemaker services, sitters,
personal care assistance, skilled nursing, consumable and durable medical
equipment, chore assistance, environmental modifications, and dental and
hearing care.
Home Health Agency -- Licensed by the State of Florida and
Medicare/Medicaid certified, LHANC’s Home Health Agency provides medical
equipment, personal care services, skilled nursing care, and homemaking
services, as prescribed by a physician to eligible homebound individuals in
the community.
