How to Request Services
Eligibility:
All services offered by LHANC have
specific eligibility requirements in order for individuals to access them.
You must contact each individual program staff in order to determine
eligibility into the service requested. All clients must be registered to
receive any of the services requested. For LHANC registration, please
contact (305) 858-0887 and specify which program you would like to register
for.
Health Services through
the Pro-Salud Clinic or Mobile Medical Unit:
Health services are offered to those individuals who are:
• A registered client of the agency through any of its congregate meals
programs
• An individual with Medicare on a Fee-for-service plan
• An individual who is part of a LHANC-contracted HMO

Adult Day Health Care
Services:
Adult Day Health Care services are available to seniors aged 60 or older.
ADHC services are available through a multitude of funding sources including
Evercare, Channeling Project, and private pay.

Social Services:
Social services are available to seniors aged 60 or older and who are
registered at LHANC. Eligibility is further using the screening tool
provided by the Florida Department of Elder Affairs. This program has no
income requirement for participation and is regulated by the Older Americans
Act.

In-home Long-term Care
Program:
Individuals that meet the following criteria are eligible to receive
services under the Nursing Home Diversion Waiver:
• Age 65 and over
• Dually eligible for Medicaid and Medicare Parts A&B.
• Live in the authorized program areas (for LHANC, Miami-Dade, Broward, and
Palm Beach Counties)
• Be determined by the CARES (Comprehensive Assessment Review and Evaluation
for Long-Term Care Services) unit at the Department of Elder Affairs to be
at nursing home level of care and meet one or more established clinical
criteria.

Congregate Meals
Programs:
Congregate Meals services, including transportation, recreation, education,
and congregate meals, are available to seniors aged 60 or older and who are
registered at LHANC. Eligibility is further using the screening tool
provided by the Florida Department of Elder Affairs. This program has no
income requirement for participation and is regulated by the Older Americans
Act.

Home Delivered Meals
Program:
Home Delivered Meals are available to frail homebound elderly persons 60
years of age or older or disabled adults who are registered at LHANC.
Eligibility is based on the individual requesting the service being
homebound. Eligibility is further determined using the screening tool
provided by the Florida Department of Elder Affairs. This program has no
income requirement for participation and is regulated by the Older Americans
Act.

Rainbow
Intergenerational Child Day Care:
Rainbow Child Care Services are offered to children between the ages of 2
through 5. This program is offered on a sliding scale fee and subsided
services are also available.
